Friendly Reminder: All students in Kindergarten, 2nd, 6th, and 9th grades are required to have a dental examination and submit a completed Illinois Dental Examination Form. The completed form is due by May 15, 2026. Please note that the dental exam must be performed within 18 months prior to the May 15 deadline.
