Good afternoon. We hope this message finds you well! We want to remind you that all 8th grade students in the State of Illinois are required to have a vision screening each year.
To make this process easier, we have the Glasses to Classes program available, which provides screenings at school. If you prefer to have your child screened elsewhere, please let us know by filling out the opt form below. However, if we do not receive an outside screening report, our district nursing staff will conduct the screening at school.
We highly encourage you to take advantage of the Glasses to Classes program! Please contact us if you have any questions

C-WC #5 is thrilled to be able to provide The Optical Academy Glasses2Classes Optical Screenings for ALL students thanks to The Absher Foundation. Forms have been sent home with all students, and another round of permission forms will be sent home this week to students who have not returned one. As a reminder, all new students to the district (pre-school, kindergarten, or transfers), 2nd grade, and 8th grade students are required to have an annual vision screening.
